Stone Cobra Questionnaire History

Two questionnaires were included as part of the Stone Cobra Mail-Away Promotion: one was available as an insert in the Burstsun Special Set and the other in the Red Flaming Phoenix Fortress boxed set. As explained on the reverse side of this insert, both questionnaires were to be completed and mailed to Takara along with the complete Mysterious Ancient Document. In return participants would receive a Stone Cobra figure along with the Stone Cobra Mail-Away Promotion Congratulatory Letter.

I don’t currently have a copy of the questionnaire from the Burstsun Special Set, so please let me know if you have a scan you could contribute to the archive.
